What causes an E02 Error Code on a 4600 operator?

An error code E02 indicate a short on the 12/24 VDC power supply.
 
On current I/O Boards with the 1-24 terminal strip:
  1.  Verify any wiring on the power output connections (1,2,3,4,5,7, etc.).
  2.  Verify any strike wiring connections. Could be a defective strike or short in the strike wiring.
  3.  Other issues could be a short on the 3180 circuit board and/or the ribbon cable is bad.
On the older 4600 operator I/O boards with terminal blocks 1-12 and 13-24, it was normally caused by the Diode (across terminals 19 & 23/24) being installed backwards (the line on the diode must be connected to terminal 23/24). If the error code E02 is on only when the actuator is pushed, the terminal blocks 1-12 and 13-24 are reversed.
